The California Surgeon General recently released the Network of Care Roadmap, a report providing guidance on how to connect health care patients to the services they need, through their health care providers. First 5 Humboldt’s Executive Director, Mary Ann Hansen, co-chaired the state committee that guided the production of the report. Humboldt’s own Redwood Pediatrics is featured as a case study in the report, detailing how patients are screened for ACEs and where the gaps exist in Humboldt to providing the “network of care” that all families deserve. The study points to the community resource directory, the North Coast Resource Hub, provided by the North Coast Health Improvement and Information Network (NCHIIN), which helps fill that gap.
