In the Spring of 2023, First 5 Humboldt staff were onboarded into North Coast Care Connect, Humboldt County’s Community Information Exchange- a network of health and social service providers that have partnered together to coordinate care for community members in need of assistance and support. North Coast Care Connect is facilitated by NCHIIN (North Coast Health Improvement & Information Network). First 5 Humboldt is a full partner in North Coast Care Connect meaning community partner organizations and agencies can refer families to First 5 Humboldt’s services and supports, and can coordinate care together, with First 5 staff providing warm handoffs, electronic referral(s), and care documentation.
